Replacement Kit Procedures If you have a Totect® kit expiring and are eligible for a replacement kit, you may return the expired or soon to expire kit 30 days immediately preceding and no longer than 30 days after the kit's expiration dating. The return should be directed to the authorized distributor that you obtained your initial kit.
To claim
your Totect®
replacement kit, please follow the instructions below: 1.
Contact your authorized distributor that you obtained your
initial kit 30 days preceding and no longer than 30 days after the kit's
expiration dating and request a RGA for a replacement kit. 2. Upon your request, your authorized distributor will ship out a new Totect® kit
along with an RGA for the expired kit. This ensures that your facility is never unprotected should you experience an anthracycline extravasation. 3. Securely package your soon-to-expire or expired Totect® kit. Affix the RGA to the package and ship back to your
authorized distributor within 15 days. 4. Your facility may be billed by your distributor for a new kit; however your distributor will credit your account when the soon-to-expire or expired kit is returned. Once you receive
your new replacement Totect® kit and the RGA, you should return the soon-to-expire or expired kit back to your distributor within 15 days
of the replacement. Please review the Totect® Replacement Kit Policy at www.totect.com/replacementpolicy and adhere to points 1
